Patients will receive complimentary take-home whitening at their initial examination (with PPO dental insurance) — because we love to see you smile.
INSURANCE
EVOLVE Dentistry accepts all PPO (Preferred Provider Organization) dental insurance plans to assist you with treatment payment. If you are unsure if you have PPO dental insurance or want information on financing for your next appointment, please give us a call. We’re here and happy to help you get the best dental service at the lowest cost to you.
FINANCING
We believe everyone should have access to quality dental care. At EVOLVE, we have financing options available to ensure you receive the dental treatment you need with convenient payment options that work for you.

When you enroll in the EVOLVE+ Membership Plan, you’ll receive benefits for simply coming to your annual dental appointments. We believe there shouldn’t be any barriers to obtaining the dental care you need. That’s why we created EVOLVE+ for your added benefit.

$150 annually (per patient) +
$130 for any additional family members
